Dome Die Break Questions

What caused the start of a dome die break on the 1891-O VAM 1A2? Is there a theory or is there a most likely cause. Are there other VAMs of this type? This VAM has always fascinated me.
I may not be the best, but I do not know anyone better.

I'm going to say it was a defect in the die that was aggravated by the clash.
