I've been looking over this 1922-p and while looking at her face it seems like she had a good scrubbing. Upon further review she also seems to have some remnants of something else imprinted on her face. I've looked over the VAM's for this coin and cannot find any mention of this type of face mark or the other issues on here including what seems like a die break on the reverse.

Looks like PMD and a harsh cleaning to me.

Yep. Damage and cleaning with something abrasive.
On a die, the portrait is usually the lowest (deepest) part of the design. When two dies clash, those low areas are protected from damage. This is why we find clash marks almost entirely limited to the field. When a coin is struck, the lowest parts of the die produce the raised design.
