fun NGC holder error (1921-S)

NGC got the date right:

It's weaker in hand than how boldly I got the lighting to make it look, but there's really no mistaking this:

Since I'm totally out of my element trying to identify an S mint that doesn't have a BU scratch or a Thornhead, can anyone identify this VAM? There's a great gouge across UTFs 4 &5:

I saw that on a 78 S Morgan in a holder.... doesn't the S$1 stand for silver dollar? Wouldn't the MM be listed right after the date?

Just to clarify. This is how a NGC 1921-S holder should look like. Dennis's NGC holder just needs an extra "S".
