We tried to identify this one a couple of months ago. Thank you JB for your help with this one. Thanks to everyone else who helped with this one. Turned out to be pretty interesting.

You might want to re-scan the letter from Leroy, covering up your address. You can edit your initial post and upload the new picture and delete the old one.
