Based only on the telegrams I've seen in Mint archives, errors on receipt were common. There's a thick file box of coded telegrams at NARA in Philadelphia. I went through some of them, but found nothing out of the ordinary. (I wish they'd tossed these and kept all the die records !)

But.....It's the very, very rare 1878-O. Obviously a pattern and worth thousands. It's also the famous "VAM 0" variety -- says so right in the VanAlien-Malice biblical thingy..... !

PS: The coin shown in the link is also a counterfeit.
