I don't know how many times the mint worker do adjustment on the press machine. But with an average of 200k coins struck per die pair, the answer is yes.

Maybe it's a 5 and a small u.

I highly doubt the mint possessed a tiny "5" and a "u" shaped die stamps; then stamped them into the working die to create that S mintmark.

Imo, that's an error due to ejection impact or die slide when the coin was ejected right after struck.

The mint polished the die when they prepared them. Unfortunately, sometime they polished it a bit too much. The result is some devices got thinner. Compare 85-O VAM-4 vs VAM-1C.
