Possibly hard planchet (poor annealing) at normal pressure. Common problem at NO Mint. Insufficient annealing capacity to balance HQ coinage demand.

Based on the photos, the odd defect is likely post mint damage – possibly intentional. The key to determining post mint versus strike through is the “Y” in Liberty. Planchet defect. Likely created when an inclusion of copper oxide (scale)was rolled into a strip, and a blank cut. The label is misleading in that the coin was struck normally and fell apart sometime after bagging.
